If you do a google search on prescription papers or similar you can find pre
perforated forms in various layouts to fit on 8.5x11 paper.  Also check out
the offerings from some of the check paper suppliers, they are perforated in
varous layouts.

The size of the paper is about the same as a small postcard. I have not 
seen any laser printers that will handle that small small in both height 
and width within the tray. Those with manual feed trays generally won't 
hold very many originals.
