I made Dennis change the name of one feature. What is now called "Mixed
Double Indenting" was once upon a time called "Downall Double Indenting."
That's where R:Style doubles the indent under certain key words within a
continued command:

SELECT +
    a, bunch, of, stuff +
  FROM table1, table2, table3, +
    table4, table5 +
  WHERE this = 'true' +
    AND that IS NULL +
    AND whateverelse +
ORDER BY this, that, theother
