Claudine, I would look further to...
When did the problem occur?  One week ago two weeks ago...What changed in the 
program at that time?Were there any changes that occured to the structure prior 
to the problem? ie...Changes in columns or names of columns that could have 
caused duplicates.

Next, I would strip the backup program to the bare necessities and perform 
those first.  I would eliminate indexes then I would perform a manual unload 
and reload, does it load properly.  If not, then I would be looking to see 
about the structural changes or index problems associated with the columns.

Thanks Bill.  Unfortunately, this exercise was meant to root out what might 
have happened.  The autochk yielded no errors and the backup failed.  I had to 
reload from a previous good backup, made (only) an hour earlier…

No, I wouldn't remove the pack index from your backup routine. I believe the 
culprit lies elsewhere. Probably in either Rbase freezing during a save in 
forms or a save in table modification. 
 
After you ran pack index, do an autochk full and a reload and you're probably 
good to go.

Very interesting…  My form names are there but not the Table/View associations 
or the Description fields but ONLY in the database explorer, everything is 
there in the actual sys_forms3 table...  The backup is unusable, output file is 
35K and lob file is missing altogether.
 
I issued PACK INDEX at the R:> just now and things appeared back to normal.  I 
had already restored the rows in SYS_FORMS3 table earlier after Karen’s and 
Gary’s messages.
 
The remaining mystery is that I have a PACK INDEX line in my backup routine.  
Could this be the culprit and should I then remove it?

I'm sorry for coming into this late. I had the same problem (disappearing form 
names). Don't know what caused it. I seem to recall that I did a Pack Index 
command and it brought back the forms. If that didn't do it, then a reload 
definitely did.
 
See if it works for you.

I am hoping that someone has experienced this and can shed some light on why 
this has happened and maybe how to resolve it.
 
This is the second time this has happened to me.  Both times, I recovered the 
database forms (listing) by restoring a previous backup.
 
Out of the blue, in R:BASE Explorer, Forms tab, the form names are listed but 
the Table/View and Form Description columns are blank.  When starting the 
database, an error message warns the user that the main form cannot be loaded.  
Same error when I try to DESIGN the form.  The form is based on FormTable which 
exists and has only one row.  An UNLOAD ALL backup yields a very small file 35 
K and no LOB!
 
Upon close inspection, the SYS_FORMS3 has all the forms with their respective 
table/view, a description and the LOB data INTACT.
 
Thinking that I could UNLOAD the sys_forms3 data, delete the sys_forms3 table 
and reload it from the backup, this is what I did and the resulting errors.
 
SET NULL -0-
OUTPUT SYSFORMS3.DTA
UNLOAD ALL FROM SYS_FORMS3
OUT SCREEN
RUN SYSFORMS3.DTA
-ERROR- You cannot delete a system table. (  10)
-ERROR- Duplicate database name ( 720)
 Database Exists 
-ERROR- Illegal table name - SYS_FORMS3 (2037)
 Begin R:BASE Data Loading for SYS_FORMS3
 End R:BASE Data Loading
 
Even with the errors, I ended up with TWO ROWS for each form, clearly visible 
in the R:BASE Explorer | Forms tab.
 
DELETE DUPLICATES FROM SYS_FORMS3 USING SYS_FORM_NAME
136 row(s) have been deleted from SYS_FORMS3
 
I am returned to a Forms List with no Table/View or Form Description in R:BASE 
Explorer.
