What you can do is  check if the ID's are unique in the source table
sys_forms3.

I don't why but I have had the same experience once and while in the stored
procedures table.

It appears then some ID's were not unique.

Made cosmetic changes to many of our forms in a development database.
Unloaded all the forms (couple of hundred) and Uploaded them to the actual
database with no other users on. An error message flashed by about the
SYS_Forms3 ID numbers needed to be unique. Database will not load for Run or
Design even though Autochk Full and Pack show no errors. The sizes of the
forms are still there. Immediately replaced the database with the backup so
everything is fine, but I had never seen this before. Unloaded the damaged
database to a very small lob file that will not rebuild. I obviously messed
up something. Is there a limit to how many forms can be Un/Uploaded at once?
It is the only significant thing I did.
