I had the same error message(your#1) just the other day, did another rebuild 
and 
did not see the message.

Did a complete rebuild over the weekend and have consistently received 3 
messages I am not sure how to fix:
1.       ‘Values for rows in Sys_Forms3 must be unique’  - I assume this means 
duplicate form names exist, but I do not see any.
2.       For the Audit table – ‘Incorrect number of values for this table’  - 
Number of rows is 10,096 and the next autonumber for AuditID is 10,097. Moved 
table to Excel, all the numbers match up and no dups. 

3.       For the Audit table – ‘Column AuditID must be a valid integer’ – 
Column 
AuditID is set as an Integer, is a PK, autonumbers every time, and acts like an 
Integer in Excel. I dropped all rows, redesigned AuditID as an Integer again, 
and started over. Same results.   
Otherwise, database operates like it should. Autochk Full shows no errors. 
Reloads show no performance changes.  R:Scope has no issues that I recognize. 
Using 9.1 64 w/ 8/22 updates.
