During our trouble shooting of this event, Jeff (our Network Admin) found that he was the first to log in (as user 0), all would be fine until he would log out, then when another user became user 0 and then logged out, problems would start soon after. Existing users would continue to be OK, but new users could not launch the app.
Using multiple Citrix servers helped us figure out what was happening to us because one server (DB and Apps) continued to run as expected while another would hang, and we could watch closely the chain of events. In our case, it's Win 2008 on the terminal server and Win 2003 on the file server, and that would cause a default SMB level of 1. The symptoms described in that link are EXACTLY what we're seeing.
