Hi Dennis,
When  do the first step, I get a file called notegen_forms.frm and 
notegen_forms.LOB
Do I need to move both of those files to the directory where the database is 
stored?
When I did what you told me, I still have no forms.

In the backup, highlight all the forms with Ctrl-A
Right click and click unload (careful!)
 
Open the reloaded db, right click where the forms should be, load, pick your 
file, bingo!

Hi all,
I was working with my application and database.
It started acting flakey, so I reloaded the data.
Now all my forms are gone!
I have a back up so I can replace them, but how does that happen?
Is there something I can do with the sys_forms table to restore them?
