Have you used the Form: Document Custom EEPs to see every eep in the form?
If that doesn't show anything, check the table for triggers. I've shot myself in the foot with them on occasion. ;)

Took the code out of the form and created a test eep using RBEdit to run by itself.
Same problem.
Something strange here.
Cannot figure out how it is calling the other EEP.
I always thought EEP's were embedded in the form, but I am running this from RBedit.
