Bob,

Make sure that the "Default State" of Check Box Object 
Properties is checked.

You can specify the settings for a check box on a form. 
You can place a check box when you want to use one of 
two values in the form; one value when the box is 
checked, another value when the box is not checked. 

Simple Steps:

01. From "Place Value Chosen into," select "Column" or 
    "Variable" 

02. Then select the column or variable in which you 
    want to place the value.

03. In "Value if Checked," enter a value that will be 
    used (returned) if this check box is checked.

    In "Value if Unchecked," enter a value that will be 
    used (returned) if this check box is not checked.

04. In "Default State," specify the default setting for 
    this check box--checked or unchecked.

05. Click the OK button to save Check Box Settings.
