Check the var sqlcode after your insert and make sure it is 0.

If there is a problem w/ the insert, the sqlcode will be something besides 
0.  If you are inserting null values, the sqlcode will be some value other 
than zero also, you may want to check that value also. (I don't remember 
what that value is, but it takes about 2 seconds to figure it out.)
