Jim,

Check Boxes on a form are useful when you want users 
to make true/false or yes/no selections. 

When you define a check box, you specify a value that 
will be used if the box is checked, and a value to use 
if the box is not checked. 

In your situation, after the initial entry form with 
check boxes, etc., to collect data, I would create 
another form (Edit Form) to display the same data in 
a column field with appropriate field settings!
