Actually, there are 4 ways it spreads. One is through unpatched Outlook 
email viewing.  One is through opening the attached EXE from any 
Windows client, not just from Outlook. One is from viewing an infected 
web page with your browser and without up-to-date Antivirus software. 
One is from running unpatched IIS.
