Jim,

No problem here using The Glorious R:BASE 2000 (ver 6.5++), 
Build: 1.842 and Build: 1.844 for Windows! 

In reference to using CHKBOX, I would recommend to use 
CHKBOX n option.

Where "n" is an optional positive integer specifying the 
maximum number of options on that menu that can be checked. 

In your case, you can use CHKBOX 1.
