You have duplicates or nulls in the primary key column.

EDIT * FROM PKtable where PKColumn IS NULL
SELECT PKColumn,count(*) AS HowMany +
  FROM PKtable +
  GROUP BY PKColumn +
  HAVING COUNT(*) > 1
