Add "FORMATTED" as the last item in your choose command.

I have been following this dialog to see if I can get the columns to line up
on the menu box.  Below is what I have tried but the second and third
columns still do not line up straight.  For example, on a given line, if the
first column is only 4 letters, then the second column is further to the
left than it is on a line where the first column is 10 letters.

If there is something I missed please give example.
I did not understand the directions to add FORMATTED to the CHOOSE command.
