set null -0-
out formname.frm
unload data from sys_forms2 where sys_form_name= 'formname'
out screen
You will get 2 files:  formname.frm and formname.lob
to put this form into your active database:
del rows from sys_forms2 where sys_form_name='formname'
input formname.frm

If this is a dos database, then use: unload data from sys_forms where
sys_form_name8='formname'
