I am in the initial stages of building a database
system for a medical firm where the staff will use
Compaq iPaqs to capture patient data in the field then
sync that data to the database at the end of the day. 
The iPaq runs Internet Explorer so I can use web forms
for data entry.  Since there is no wireless connection
in the field yet, the staff will sync up when they get
back to the office or from home over a VPN.  The
patient and scheduling info they need for the next day
will be pushed out to them as well.  The medical staff
will be able to record audio notes on the iPaq which
will be uploaded to a transcriber who will enter the
text in the database.

This solution is only for data input since the
database will not reside on the PDA but it is a step
in the direction you mentioned.  A fast, reliable
wireless Internet connection in the field will truly
revolutionize things but that is years away.
