At the R:> type FORMS and then choose your form and enter the description in the box provided then you MUST edit the form and save it - it is no good quitting the Forms Designer box without entering the form and saving it.
Same goes for reports. According to R:Syntax COMMENT ON does not include forms and reports at present but I guess it will be in v7.
