Using the "Page Style" option of R:BASE 7.1 for Windows,
it should be a breeze to design the HCFA-1500 Claim Form.
While submitting the form it is also important to note
that there might be cases when there are multiple
modifiers assigned to a code.
If there are multiple modifiers assigned to a code,
CPT contains modifier -99 which is used to alert third
-party payors that several modifiers are being used to
report a service. The modifier -99 states "under certain
circumstances, two or more modifiers may be necessary to
completely delineate a service". In such situations,
modifier '-99' should be added to the basic procedure
and other applicable modifiers may be listed as part of
the description of the service. Modifier code 0999 may
be used as an alternative to modifier '-99.'
For example, if a surgical procedure is performed using
an assistant surgeon (-80) and the physician elects to
eliminate a portion of the surgery (-52), the service
would be reported by one of the following methods:

In situations where more than one modifier needs to be
recorded on the claim form, individuals may also be
advised to contact their Medicare Carrier/Intermediary.
Once you have everything under control and designed
accordingly in R:BASE 7.1 for Windows, you have the
following two options:
01. Duplicate the entire HCFA-1500 Claim Form in R:BASE,
using "Page Style" option of Report Designer.
02. Use the LASER HCFA FORM 1500.
