I restored a copy of the database from yesterday and the format 000-000-0000;1;_ worked properly.  I then unloaded the form from the restored DB and reloaded it in the failing DB and it now works properly.  Somehow the format must be corrupted in the forms table.

Hmmm...I used to have the same problem, but I was not up-to-date on the latest version.
What I did was go to the format help screen, and used the cut and paste feature to pop it into the format field instead of typing it, and it seemed to work after that.  I don't know why, but it happened a couple times.

One of my clients called today and said that they are getting errors when entering telephone numbers.  I investigated and the problem and found that the format mask 000-000-0000;1;_ produces an error (The form or table has not been modified in six months).  I tried changing the mask to 999-999-9999;1;0 and it fails but with different symptoms.  As soon as you enter the first digit it skips to the next field.
 
I did add a form to the database this morning but it does not use or modify any of the fields or tables used by the failing form.
 
Any idea what may be causing this problem?
