There are a few possibilities. I'll start with the most common first.

Are you getting the lock nut tight enough? Are you using just one
wrench? Properly tightening a threadless set usually requires two
wrenches to do the job well. Use one wrench to hold the lower,
threaded race and the other to FIRMLY tighten the lock nut against it.
I use a big crescent wrench on the lock nut instead of a thin headset
wrench to avoid slipping and marring my headset as I apply sufficient
force.

Another possibility is that the lock nut doesn't have the proper
spacing from the top of the steer tube. Did you remove any spacers or
install a different cable hanger for your canti brakes? The lock nut
needs a couple of mm of space from the top of the steer tube in order
to be tightened properly.
