Cotton is a poor choice for cycling underwear in most circumstances. I've used 
the RBW sold Norwegian wool boxers as well as British army surplus undyed wool 
boxers as liners under unpadded shorts with good results. I still apply Bag 
Balm for both its anti-microbe and anti-friction qualities.
For liners, I've really liked Zoic "assault" units that come in their mountain 
bike style shorts like "Black Market" and "Ether". They are sold seperately 
too: http://www.zoic.com/assaultliner  I think they are hands down better than 
Andiamo and similar. They work well under MUSA shorts and you don't see the 
"diaper".  The Zoic shorts are very comfortable as well and are in a rotation 
with MUSA. 
I wear liners when breaking in new leather saddles and on rides over about 30 
miles.

I don't like having to wear bike diapers, but admit that I haven't had this 
problem since switching over to the REI Novara liners underneath MUSA shorts.
