Ryan, it attaches to braze-ons on the fork/drop-outs (like most front racks) and to the fork crown. Matt designed this piece that attaches to the rack that slides back and forth and allows you to level the rack. That bit attaches to the fork crown. It probably wouldn't be too difficult to rig something to use a fender mount as well though.
