I wouldn't ride a Alba or similar bar without them !   You can mount 
Paul thumbies next to the lever if you wish.

I prefer thumbies around the inside top inside myself as my hands are most 
often in the middle on up, almost never all the way back. I usually brake 
with just 2 or 3 fingers, the smallest ones . 


Alternatively, Jtek makes some levers that mount into Shimano bar end 
shifters, and reverse brake levers that clamp on(22.2) that you can mount 
just after the bar end shifter.
