I had the same thing happen a few years ago with the carbon fibre rim on my 
rear wheel. In my case, what punched through the rim looked like a piece of 
broken motorcycle spoke that got flipped up by my front wheel and went into 
the rear, punching about a 4mm hole through the tyre, the rim bed, and 
finally exited the rim midway between two spoke holes. I was able to ride 
home with a new tube, and as the damage was midway between spoke holes, I 
just used a bit of epoxy filler to patch the holes and fitted new rim tape, 
and I'm still using that wheel with the same rim.
