Eric, I have had a couple of crank arms looses...it has always been my fault.
When I torque the crank-arm bolts I have not had one come loose. I torque to 25-28 ft-lbs although the recommended value varies from crank to crank and non-square drive cranks (which I don't own) are probably different. I grease the BB spindle tapers prior to installing the crank arms; opinions differ on this point.
