I haven't resorted to wider cranks yet as I have the perfect cranks (TA Zephyr) and am hoping to avoid that! But I have done the roller (but not in a while), stretches, and lowered saddles a bit. The big help was angling cleats so my heels were really in (toes pointed out), and also pushing the cleats inboard as far as possible, which in effect widened my Q by a couple mms. All that has limited the onset of inflammation and pain considerably. I can't ride without foot retention, in fact that's what caused my initial onset. Now I only have a flair up on fast-paced, high-medium mileage road rides for the most part. I can ride 40 milers now w/out incident. Haven't rode longer than that in a while to see how I do, but I imagine it would start flaring up. That's one of the reasons I favor mixed-surface and MTB rides almost exclusively now. The variation in pedaling and movement (and stopping to catch my breath!) really helps out. Sitting in the same spot spinning on the road is NOT good for me.
