Is it just one knee or both?  Maybe you could put a kneesaver on one
side or use an Assymetrical BB to move the troubling side out a bit?
I also pedal heel in so I know what you mean about getting the cleat
angle right!  Anyway, it's a frustrating problem--hope you find
relief!
