On mine, the chain gets caught between the middle ring & chainstay. When this happens, the chain is on the inner ring at the top but the section that's disengaging from middle to inner gets stuck between the stay & the middle. Visually, there appears to be more space between the inner & middle rings than the middle & outer rings.
Mine is an XD with flat rings (no ramps or pins), with a 110 mm BB. I'd thought that next time I need a BB I'd go up to a 113 to give the chain more clearance with the stay.
