Here's my favorite bike bag:

Has two belt-loops and four spring clips on the back.  I added a 1" key 
ring to each belt loop, can fold the top clips around the bars to clip to 
the rings.  You can run a strap from the two bottom clips, behind the top 
tube, to keep the bag from swaying.  The side posckets are large enough for 
a standard water bottle.  I've also used it clipped to the bag loops on a 
Brooks saddle.

This one's had trim added, and is stiffened back and bottom with an old 
3/16 thick plastic cutting board.  It's mounted to the bike with a 
decaleur, and a strap around the headtube.
