In this particular instance, by not including the Web and/or
e-correspondence with authors and publishers as sources of
information.
  
We find, for example, the Web a frequent source of the location of
publishers for place of publication.  Internet search for the
publisher's website, and click "Contact us".  Often cataloguers of
DVDs do very complete complex records, but neglect that quick and easy
step, giving a "not identified" phrase or a guessed country.

RDA seems to assume (as in days of yore) that we are limited to print
sources, unless one considers Wilipedia a "reference source".
