Have you tried calling .reset() on your loanApplicationIterator? or
maybe the useraccount object is in a persistent scope and the iterator
is not being refreshed

Just curious, Are you always using getat(1) on your Iterator, or are
you looping through? Also last straw attempt:are you sure the problem
isn't that an existing record is being updated when calling save,
rather than a new one?
