FWIW- and this may have been suggested on the blog-

Apache has pretty good logging facilities.  Sounds like an access problem,
in which case, the access logs would be a good place to look when things
aren't going the way you want them to.

You can get logging on pretty much anything apache does, which takes
a lot of the uncertainties away (you can control the level as well).

As I said, maybe already suggested, and fruitless in this case, but it's served
me well every time I've had that type of problem.
