What version is this? If it's 5.5, the problem is that Document Icon
is checked for a file type that doesn't have an icon (which is a bug
in 5.5). However, if you can see it in 2005/6, I'm a bit puzzled.

It's occurring in RB2006r2 on OSX 10.4.6
The project started life back in 2001 in the then current version. It
did live in RB 5.5 for quite some time, and it's been working fine in
RB2006 since the first of the year. (Carbon PEF, Classic and Windows)
I'm now attempting to compile for Mach-O and consistently encountering this FA.
I think there are FileTypes with the DocIcon checked (to get proper
drop behavior) but without an icon defined (don't want to redefine
the icon for another app's filetype)
At least now I have an area to look in. Any other things to look for?
