Ah, I see.
If I delete the file, restart Finder, open the app and create a new
file, it now shows the new updated icon.
Two last questions.
1. On normal documents, after editing part of it so it has unsaved
changes, it shows the Close Button as a Circle. Is this possible
using RB?
2. In document windows, the icon for the document shows just before
the title name. How can I add this icon using RB?

No.
The Finder caches things which is why a restart of the Finder makes
it show.
There is a way to make the Finder refresh but I dont recall the
specifics off the top of me head
I believe there is an Apple Event you can send the finder to make
it refresh
