Yes, I ran into this a week or two ago.  The problem was very
persistent, until I quit RB and restarted it; then it went away and
hasn't happened to me again.  I have no idea what caused it.  Which
means that it's a real problem, but may be very difficult to track
down.  I'm sorry to say that I didn't file a bug report, hoping that it
might happen again so I could have something more useful to say about
it.  In retrospect, I probably should have filed it right away.

(I did talk to an old friend at RS about it, however, and learned that
they had not seen nor heard of this behavior before.)

I believe this is a new bug.  A new bug report is needed.

I note that most of my projects use external items in XML format.  The
one where the problem was observed uses external items in regular
(.rbo) format.  From this one data point, I tentatively hypothesize
that this bug only affects .rbo external items.  Can you support or
refute?

Let's not devolve into whining.  They haven't seen this bug; most other
people haven't seen it, either.  Let's work with them to help them nail
it down, rather than starting a whine-fest about how RS is evil and
personally out to make us suffer.
