I have had this happen with the Windows IDE (2007r1), with classes that I
use in different projects. I made changes to these classes in one project on
my Mac at home. I brought the files to work, where I use them in a different
project, also as external items. I replace the files on my Windows machine
at work with the updated files I brought from home. After opening the
project, everything seemed to work fine. I made some more changes to the
external classes and saved the project, when I noticed that the file date on
the external items didn't change. They were not saved. After closing the
project and reopening it, the changes I just made were gone. Even after
reopening the project, changes to the external classes still did not get
saved.
The only way I was able to make the IDE save the external items was to
remove them from the project and re-add them. After that, the external items
were saved as expected.

The status on this issue says "fixed/implemented". This should probably be
re-opened.
