I think you're out of luck here until Leopard is released. It will allow your application to register with the Instant Message framework and receive Core Video and Core Audio callbacks. I may be wrong, but this framework isn't available in Tiger as far as I know. Skype video integration will be possible if the Skype devs decide to make their app Instant Message-enabled.
