The RealProducer ActiveX control is how this is done.  You can write a
VB application that runs the RealProducer and inserts URL events into
the live stream or even embed the RealProducer ActiveX control in a web
page and use  VBScript to trigger insert events.  The function call to
the Realproducer is very simple, (something like SendURLEvent(URL)).
