The answer is that it depends on the bitrate that you specify.
In the stream header, there is a field labled "AvgBitRate"

The rate that GetPacket is called depends on the number of bytes that
you put into a packet.  For example, if you have a bitrate of 800 bits
per second and you put 10 bytes into each packet, GetPacket will be
called 10 times a second (assuming 8 bits to the byte).

When streaming the RealServer will try and stream data at 110% (I think)
of your specified bitrate.  It assumes that there will be some latency
or packet loss, etc. between the player and server.

The packets are recieved by the rendering plugins at as close to the
specified bitrate as possible.
