If you are using 0.5 rexserver, then you only have standard realxtend
voice chat.
That thing is buggy and only works in single region. Works only with
0.42 client.
On positive side, 0.5 realxtend voice chat is simple and it should be
possible
to fix problems in source code

The new Naali/Taiga mumble-based voice chat should works much better
and support
multiple regions. Although (in my opinion) Naali is not yet ready for
the kind of application you
have in mind, it might be  ready by the time your project is finished.

This sounds large. No alternative to grid mode, and you should switch
to Taiga, as it is more
stable and you will probably need http assets downloading feature.
Realxtend does not currently
support large continuous regions (but pure opensim does).

There was a problem with rex authentication in grid mode, and thus
problem with avatar configuration storage.
Last time I checked, the problem was still there, but this might be
fixed in latest versions. Realxtend  project is going forward really
fast, but I think that developers have no time to test large-scale
configurations in real working environment. So, if anyone has
expirience with running Taiga grid with multiple regions and rex
authentication, I would like to hear about it too.

That said, I have succesfully run 1.5 sq miles content-rich simulation
with old releases, so this should be possible, while exact
implementation strategy depends on your specific requirements.
