We use FTP publication and CMS will create any subfolders it needs without issue. I've seen differences in FTP default folder paths using an FTP program vs. CMS which I fix by including the root path in the FTP address. Is CMS possibly starting outside of the FTP user's home directory? Maybe set the FTP address to ftp://server/username?
