It sounds like you are missing a step. Don't you need to use a reference dynament to first upload the file?
The attachment dynament does allow you to send a named file as attachment but the ref attribute must specify a filepath liveserver can recognize. Sorry I don't have a ready example but you should look up the manual on reference dynament.
