Then it is possible that a page is connected multiple times, mainlink
location and via category and keyword at another location.

Though the page is not rendered (and consequently published) in the
page connected via category and keyword due to the "Maximum number of
pages" restriction, the page can still be published from the mainlink
location or simple published from its ancestor page with the option
"publish all following pages" enabled.

A technique call "shadow reference" is often recommended to ensure a
page is only publishable from a single location despite its many
connections.
