Check the cleanup logs to see if there were errors. In IIS 7+ the directories controlled by IIS are protected. Depends on how your ACLs and UAC is setup as to whether the system lets you do things in IIS directories or whether it forces you to do things through the UI (with prompts). Maybe try Process Monitor to see if the cleanup agent is trying to run but being denied access to the files...
