Usually you just run setup.exe. This should do what you expect. (Upgrade 
your system inplace.) We started initially with version 5.5. With 11.0 we 
migrated to a new windows server 2008 R2. But we still use our existing 
project databases. We installed some hotfixes for 11.0 during time and made 
the upgrade to 11.1. The upgrade done by setup.exe always worked in our 
environment fine.
After the upgrade you have to update your project databases via server 
manager. (I expect you know this already.)
A well know best practice is to make backups of your system before 
updating. In our case we have virtualized servers running within VMWare 
ESX. So we make snapshots of the server before we update them. (We never 
had to step back to one of this snapshot´s, but it´s always better to have 
some kind of safe net.)
