Hmm ... off the top of my head, the following conditions would cause
..bash_history to not be modified: a) not logging in; b) changing the
shell to something other than bash; c) changing the HISTFILE variable;
d) a value of * for HISTIGNORE; e) HISTCONTROL=ignorespace and you
habitually space before typing a command.

Or maybe something's really broken.  =-)
