I'm still curious.
I haven't a clue as to what causes the db files to be created in the
first place. I've checked 4 machines, 2 running RH 7.1 and 2 running RH
7.2. None of them contain any files in /var/db/. I assume that the db
files are created by another program, and unrelated to useradd and
friends. What program access those db files?

What is the output of rpm -qf /var/db/Makefile (or any other file that
exists in that directory)?
