If you're lucky, as I was with my Supra 28.8i, this may work:

Check which com port your modem is on in Windows, assuming you have that,
and that it is being set pnp style there.  Write down the irq # and the
i/o address.

COM 1 = /dev/cua0, COM 2 = /dev/cua1, etc.

Then try this, as root.

setserial /dev/cuaX auto_irq autoconfig

Then do setserial /dev/cuaX to see how the resources are set.  If they're
the same as in Windows, then things are looking good.

Next try to dial out with minicom.  If this works, you have your modem
resources set right.

If this doesn't work, then you'll have to try something else.
