Record deprecated bnodeid functions. Adjust macros.
Add RASQAL_DEPRECATED for
rasqal_query_set_default_generate_bnodeid_parameters and
rasqal_query_set_generate_bnodeid_handler.
If RASQAL_DISABLE_DEPRECATED is defined, remove all deprecated
prototypes and typedefs.
